Chronic Condition Patient Self-Management

Patients with chronic illness need support, as well as information, to become effective managers of their own health. In order to meet these needs, it is essential for them to have the following:

  • Basic information about their disease
  • Understanding of an assistance with self-management skill building
  • Ongoing support from members of the practice team, family, friends, and community

ScoreHealth is a physician/nurse-educator team program that focuses on identifying risk factors for chronic disease. ScoreHealth services are provided at each of the Peninsula Primary Care clinic sites and appointments are scheduled by your family practice office. Services provided include an initial screening/risk assessment for chronic disease conditions and behavioral risk factors. Specific education and evaluation is then provided for a patient’s particular diagnosis including heart disease, high blood pressure, high cholesterol, diabetes, obesity and depression; and behavioral risk factors including stress management, smoking cessation, physical activity and nutrition. Goals are made and a consistent follow up schedule planned. ScoreHealth is part of your regular doctor’s office visit and is covered by your insurance. You will receive a personal plan for making healthy changes and will have the time necessary for your questions about health to be answered.
